Abstract

Ageing migrants living in Adoma social residential homes are often in a very vulnerable situation because they are too often overlooked by the authorities, social services and society. In the Rhone-Alp region the Intermed association which is a health network recognised by the regional health agency (ARS) has set up, on the initiative of Adoma Rhône-Alpes, a "health mediation and coordination" mission for the most isolated residents with complex health/psycho-social situations, and especially for older residents. The Intermed nursing staff, under the supervision of a social coordinator, work on the basis of relational care so that the most vulnerable people find favourable conditions for accessing care but also for social reconnection linked to their environment and with all due respect to their individual projects. Where care and cure meet, an interview with a kind and attentive pair. © Fond. Nationale de Gérontologie. Tous droits réservés pour tous pays.
